White Spots - Incidence, Prevention and Treatment.

Decalcifications described as White Spots are well documented side effects of orthodontic treatment. Incidence of 10-70 % has been reported in the literature depending on registration criteria. Recent studies using a criteria visible – nonvisible W.S. have reported an incidence of aprox. 25 %. Fluor application plays an important role in primary and secondary prevention with a high evidence level. Studies have reported regression of W.S. up to 6 month after removing the orthodontic appliance indicating a remineralization process in place. Although these findings have strong individual variation much interest has been focused on stimulating this remineralization process in both primary and secondary prevention. Agents has been evaluated and recommended in various combinations with / without Fluoride with promising results but still with insufficient evidence levels. Treatment of persisting W.S. have been described in the literature using different techniques such as micro abrasion and recently resin infiltration reporting positive results. This presentation will shortly discuss the caries etiology and evaluate means of primary and secondary prevention from a cost benefit and evidence based aspect and review future options in prevention and treatment of white spots.
