Registration

Early registration until 30.6.2014.

Congreess is registered by the SKZL and participants will get 8 credits for each day, 24 credits for all three days. There is no registration by the Czech dental chamber due to the conditions of the Czech dental chamber.

To register for the Conference, please complete the On-line Registration Form  

Please note that the on-line Registration Form is only accessible with a password. Simply fill in the Personal Data Form and a password will be sent to you. Your password is the same for all on-line forms.

Upon receipt of the registration form and the payment, a Letter of Confirmation will be sent to the participants. Please present the Letter of Confirmation at the Registration desk at the Congress venue to pick up your materials.

Early registration fees will only be confirmed if the registration form and the payment are received by the Congress Secretariat on June 30, 2014 at the latest.

Registration forms received after September 8, 2014 will not be accepted as pre-registration. Please do not send any registration forms or payments after that date. You will have the possibility to register in the Congress venue. The on-site registration will be available from September 18, 2014. All participants will receive the Letter of Confirmation within three working days via e-mail or fax after the submission of their registration form.

Registration fee for caterory 6 and 7 includes:

Entrance to lectures for orthodontic assistants and dental technicians, exhibition and poster session on 18.9.2014 and 19.9.2014
• Congress materials
• Coffee breaks 18.9.2014 and 19.9.2014
• Lunch on 18.9.2014 and 19.9.2014
• Party for orthodontic assistants and dental technicians on 18.9.2014

Registration fee for caterory 8 includes:

Entrance to scientific section, exhibition and poster session on 19.9.2014
• Congress materials
• Coffee breaks and lunch on 19.9.2014

Registration for category 9 includes:

• Entrance to scientific section, exhibition and poster section on 18.9.-19.9.2014
• Congress materials
• Coffee breaks, lunches
• Welcome party on 18.9.2014
• Entrance to Opening ceremony
• Entrance to President’s party on 19.9.2014

Only registered participants will be admitted to the scientific section, exhibition, poster section and courses. Registration of the pre-registered and new participants will be located on the 1st floor. Please check your congress material upon presentation. Late claims cannot be accepted.

Refreshments

Complimentary coffee and tea will be available in the exhibition area, menu style lunches will be provided in the hall Evropa 3.

Transport services

Transport service to the congress venue and social programme will not be arranged.

Badges

Participants will receive a name badge. Everyone is requested to wear this badge during all congress activities. Cost of replacing a lost or mislaid badge: 4 EUR.

Certificate of attendance

All participants will receive a certificate of attendance at registration desk.

Welcome drink

Welcome drink will take place in the exhibition area on the 1^st floor on September 18, 2014 and is free of charge for all registered participants.

Social evening for the orthodontic assistants and technicians

Social Evening will take place in Belmondo club on September 18, 2014.

Presidents reception

President´s Reception will take place in the premises of the Faculty of Natural Science on September 19,2014. Free admission for category 1,2,3,4,5,9,10.

Exhibition area

Exhibition will take place in the foyer on the 1^st floor. Entry s included in the registration fee.

Cancellation and refunds

The Conference Secretariat must be notified in writing about the cancellation of registration by fax (+420) 284 001 448 or e-mail COS2014@guarant.cz  . Telephone cancellations will not be accepted. The appropriate refunds will be made after the Congress. The following cancellation conditions apply:

Methods of Payment

Registration fee must be paid in EUR by one of the following methods:

1. Credit card

For payments through the Online Payment System   (available after login) only MasterCard/Eurocard, Visa and Diners Club credit cards are accepted. These payments will be charged in EUR.

For payments using American Express credit cards please complete Credit Card Payment Authorization Form   and kindly send it signed by a card holder to the Congress Secretariat by fax (+420 284 001 448) not by e-mail for security reasons. The payment will be charged in local currency – Czech crowns (CZK) – at the exchange rate of the Czech National Bank valid on the date of payment. The approximate exchange rate is 1 EUR = 27,484 CZK (December 2013).

Credit Card Payment Authorization Form   can be also used for MasterCard/Eurocard, Visa (will be charged in EUR) and Diners Club (will be charged in CZK).

Bank transfer

Please note that payment by bank transfer is possible by September 15, 2014.

After this date only payments by credit cards will be accepted (you can either use Online Payment System   or send us an Credit Card Payment Authorization Form  ).

All payments made by bank transfer have to be net of all bank charges. The payer pays the bank charges of their bank, and the beneficiary pays the charges of their bank, if any.
